Waiver & Consent Form

In consideration for being allowed to participate in the Program, Parent and Participant acknowledge and agree to the following:

  • I understand that the Program will be provided via [list any platforms used, e.g., Zoom, Google Classroom, etc.], unless otherwise notified.
  • I understand that only program staff, registered individuals, and authorized individuals are permitted to participate in the Program activities, but there is a possibility that unregistered or unauthorized individuals (e.g., siblings, friends) may be present.
  • I agree to comply with the Program’s Code of Conduct and understand that violation of the Code may result in the Participant’s removal from the Program at the discretion of program staff, upon notification to the Parent.
  • If a Participant requires any disability-related accommodations, I have identified them below. I understand that if I do not notify program staff of any accommodation requests at least one week in advance of the program start date or if external resources are required, such accommodations may not be available by the start date. Requested accommodations (communicated via email or in written form):
  • I understand that participation in the Program involves risks, including but not limited to exposure to cyberbullying, unauthorized contact, and technical disruptions. I voluntarily assume responsibility for all risks associated with the Participant’s participation.
  • I hereby waive, release, and agree to hold harmless Young Scholars Circle, LLC/The Masterpiece Academy, its officers, employees, volunteers, and agents from all liability for any harm that Participant, Parent, or our property might suffer as a result of Participant’s participation in the Program, except for liability determined by a court of competent jurisdiction to constitute gross negligence.
  • I agree not to assert any claims or actions against the Young Scholars Circle LLC/The Masterpiece Academy in connection with any harm that the Participant, Parent, or our property might suffer as a result of participation in the Program.
  • I agree to defend, indemnify, and hold Masterpiece Academy harmless from any claims, demands, damages, liabilities, expenses, or losses, including attorney’s fees and costs, sustained or alleged to arise from the Participant’s participation.
Permission to Record and View Class Sessions

I understand that all live online and hybrid classes are recorded for internal use only. These recordings are intended solely for class members who wish to review key concepts. Young Scholars Circle LLC/The Masterpiece Academy does not publish these recordings or make them publicly available.

I agree that I will not share, distribute, or publish any class recordings outside of the class. All students and class members are strictly prohibited from sharing class recordings with individuals who are not enrolled in the paid class. Any unauthorized distribution of class recordings will be considered a violation of this agreement and may result in removal from the Program.
